Immunizations & Hearing/Vision/Dental/Nutrition

By state law, a valid Georgia Certificate of Immunization (Form 3231) or a notarized statement of religious exemption is required for enrollment. A valid certificate (Form 3231) must be marked with either Date of Expiration or as "Complete for School Attendance. If it is marked with a "Date of Expiration a new certificate must be submitted within 30 days of that expiration date. A medical exemption, if applicable, should be noted on the Georgia Certificate of Immunization (Form 3231) with a current date of expiration. A medical exemption must be obtained by your childs physician annually. You may request a form from your school nurse for religious exemption or you can obtain one from your local health department.

There are new vaccine requirements for students who are born on or after 01/01/2002 and entering 7th grade. The requirements also apply to students who are new entrants to 8th – 12th grade. New entrants are students who have never attended any school in Georgia for at least one full school year. Summary of Georgia Immunization Requirements for Child Day Care & School Attendance

Required by state law for enrollment, a student must have evidence of a vision, hearing, dental, & nutrition screening that has been obtained within the past 12 months of the first day of school. This information must be on a Georgia Form #3300 (revised 2013), available from the health department, or your physician. Please see the previously mentioned form below.
